The first step in any successful improvement program is to measure current conditions. Your measurement results create a baseline on which to set appropriate improvement goals and compare future measurements. A cyclical approach of measure while setting appropriate goals is the key to maximizing your health improvements.
Biometric Screenings
Comprehensive, onsite biometric screening can be provided as part of our wellness program. The screening data can then be uploaded to the Vector Wellness program to provide a comprehensive health profile. Biometric screening data may include: height, weight, waist size, body fat, BMI, hip to waist measurement ratio, pulse rate, blood pressure, total cholesterol or full lipid panel (HDL, LDL, triglycerides and total cholesterol), glucose, PSA, Cotinine (exposure to tobacco smoke), and flu-shot services.
Health Risk Assessment
Data from the Biometrics Screening events are uploaded into our Health Risk Assessment (NCQA certified) which is provided by our partner, PDHI. The completed assessment calculates the individual’s health risk related to current health status, personal medical history, family medical history and lifestyle. The advanced algorithm combines the individual’s information to calculate an overall Wellness Score which can be tracked over time to show changes. In addition to a summarized assessment, each individual will get a detailed report that rates his or her health risks and makes personalized recommendations to improve their health. Vector Wellness uniquely offers focused Health Risk Assessments for adult, college age, teenager and pediatric populations. Participants are empowered to manage their health by creating a health risk assessment report to be shared with their physician or wellness coach.
Organizations can run reports to understand the health risks associated with their population. Health Risk Assessment data is securely stored using modern technology and meets all industry standards, and federal guidelines, including the requirements of the Health Insurance Portability and Accountability Act (HIPAA) and the American Accreditation HealthCare Commission (URAC).
